Stroke หรือ โรคหลอดเลือดสมอง คือ ภาวะที่ทำให้เซลล์สมองถูกทำลาย ซึ่งมีสาเหตุมาจากหลอดเลือดสมองตีบ อุดตัน หรือแตก ทำให้ขัดขวางการลำเลียงเลือดซึ่งนำออกซิเจนและสารอาหารไปเลี้ยงเซลล์สมอง ส่งผลให้สมองสูญเสียการทำหน้าที่จนเกิดอาการของอัมพฤกษ์ อัมพาต หรือร้ายแรงถึงขั้นเสียชีวิตได้

โรคหลอดเลือดสมองสามารถแบ่งได้เป็น 2 ประเภท ได้แก่
1. Ischemic Stroke เป็น “ภาวะหลอดเลือดสมองตีบตัน” หรือ “ภาวะสมองขาดเลือด” มีสาเหตุมาจากการเสื่อมสภาพของหลอดเลือดจากการสะสมของคราบไขมัน หินปูน ที่ผนังหลอดเลือดชั้นในจนหนานูน แข็ง ขาดความยืดหยุ่น ทำให้รูของหลอดเลือดค่อยๆ ตีบแคบลง ส่งผลให้ประสิทธิภาพของการลำเลียงเลือดลดลง หรืออาจเกิดจากลิ่มเลือดจากหัวใจ หรือการปริแตกของผนังหลอดเลือดหลุดมาอุดตันหลอดเลือดในสม
2. Hemorrhagic Stroke เป็น “ภาวะหลอดเลือดสมองแตก” หรือ “ภาวะเลือดออกในสมอง” ส่งผลให้เซลล์สมองได้รับบาดเจ็บจากการมีเลือดคั่งในเนื้อสมอง ทำให้เนื้อสมองตายมักพบในผู้ที่มีความดันโลหิตสูง ทำให้หลอดเลือดมีความเปราะเเละโป่งพอง

ปัจจัยเสี่ยงต่อการเกิดโรคหลอดเลือดสมองมีดังนี้
1. ความดันโลหิตสูง
2. โรคเบาหวาน
3. คอเลสเตอรอลในเลือดสูง
4. โรคอ้วน
5. การขาดการออกกำลังกาย
6. การสูบบุหรี่จัดและการดื่มสุราเป็นประจำ
7. การใช้สารเสพติด

สัญญาณเตือนโรคหลอดเลือดสมอง
อาการเตือนโรคหลอดเลือดสมองที่เราสามารถสังเกตได้หลักๆ มี 5 สัญญาณเตือนสำคัญ ดังนี้

1. ชาหรืออ่อนแรงใบหน้าซีกใดซีกหนึ่งอย่างฉับพลัน ทำให้มุมปากตก ปากเบี้ยว อมน้ำไม่อยู่ น้ำไหลออกจากมุมปาก
2. ชาหรืออ่อนแรงที่แขนขาซีกใดซีกหนึ่งอย่างฉับพลัน สูญเสียการทรงตัว
3. พูดไม่ชัด พูดไม่ออก สับสน นึกคำพูดไม่ออก
4. การมองเห็นมีปัญหาฉับพลัน อาจมองเห็นภาพซ้อน มองเห็นภาพครึ่งเดียว ตาบอดหนึ่งหรือสองข้าง
6. มีอาการปวดศีรษะรุนแรงฉับพลัน
หากพบอาการดังกล่าว ควรรีบนำตัวผู้ป่วยส่งโรงพยาบาลใกล้บ้านทันทีค่ะ

In this lecture Professor Zach Murphy will be presenting on Ischemic and Hemorrhagic Strokes, ICH, SAH, a high yield review. We will be starting with the anatomy and physiology of the various structures and regions within the brain, along with the cerebral vasculature including the circle of willis. We then move into the etiology and pathophysiology of transient ischemic attack (TIA), acute ischemic stroke, intracerebral hemorrhage (ICH) and subarachnoid hemorrhage (SAH). Next, we move onto the clinical features of strokes and differentiating the various type of stroke syndromes and how they most classically present. The next portion of the lecture will focus on the diagnosis of acute ischemic stroke, intracerebral and subarachnoid hemorrhage, utilizing various diagnostics such as CT, MRI, CTA, and blood work. We conclude this lecture with the treatment of TIA/acute ischemic stroke, intracerebral and subarachnoid hemorrhage. We hope you enjoy this lecture and be sure to support us below!
