This is Brainbook case 3 together with the second installment of our Brainbook GoPro series.
This young man in his mid20s was referred from the eye hospital because of papilloedema in both eyes.
The case video shows CT and MRI scans together with endoscopic surgery (using a camera to look inside the brain) as well as GoPro footage of ventriculoperitoneal shunt insertion.

3 MONTHS OLD MALE CHILD PRESENTED WITH HISTORY OF GRADUAL INCREASE IN SIZE OF HEAD FOR LAST 1 MONTH
THE CHILD HAS BECOME MORE IRRITABLE AND HAS FAILURE TO THRIVE, NO H/O FEVER
ON EXAMINATION HEAD CIRCUMFERNCE WAS 55CMS AND HAD DILATED VEINS ON FRONTAL REGION. ANTERIOR FONTANELLE WAS TENSE AND BULGING WITH SUNSET SIGN.
THEIR WAS NO CONGENTIAL NEUROCUTANEOUS MARKERS
PATIENT WAS INITIALLY EVALUATED AT PERIPHERAL HOSPITAL AND SHIFTED TO OUR HOSPITAL FOR FURTHER MANAGEMENT
HAEMTOLOGICAL WORK UP SHOWED NORMAL FINDINGS
CT SCAN SHOWED DILATED LATERAL AND THIRD VENTRICLE WITH NORMAL FOURTH VENTRICLE WITH EFFACEMENT OF SULCI AND GYRI.
PATIENT UNDERWENT RIGHT VENTRICULOPERITONEAL SHUNT
REASON FOR CHOICE OF VP SHUNT OVER ETV WAS BECAUSE IN VERY YOUNG INFANT ETV HAS POOR RESULTS.
POSTOP CHILD WAS NEUROLOGICALLY NORMAL AND HAD STARTED ACCEPTING FEEDS EFFECTIVELY.HEAD CIRCUMFERNCE WAS REDUCED TO 52CMS.
FONTENELLE WERE SUNKEN AND DILATED VEINS HAS DISAPPEARED
POSTOP CT SCAN SHOWED SHUNT TUBE WAS IN SITU IN LATERAL VENTRICLE AND DILATED CISTERNAL SPACES
POSTOP PATIENT HAS MADE COMPLETE IMPROVEMENT.

Life after VP Shunt


A ventriculoperitoneal (VP) shunt is a medical device that relieves pressure on the brain caused by fluid accumulation. There are two types of VP shunts programmable and nonprogrammable. In a nonprogrammable shunt, valves are programmed to activate when the fluid reaches a certain volume. In a programmable shunt, there is an external and adjustable valve that the doctor can readjust at any time according to the person’s needs. The total recovery takes 2 to 3 weeks.
Start with liquids and move on to solid foods.
Keep the incision site clean and check for signs of infection every day.
Doctor will let you know when to start showering again.
Traveling is allowed after 4 weeks or so. Always carry your Patient Identification Card, provided by the neurosurgeon, with you at all times.

Avoid strenuous activities that involve physical blows to the head for approximately 6 weeks.
Doctor will advise when to resume normal daily activities and go back to work, which is usually after 7 weeks.
VP shunts can be replaced anytime between six years to eight years of time.
Driving: Wait for at least 6 months or can be longer if you have highpressure headaches.
Avoid bringing smartphones, toys, audio headphones, loudspeakers, shaver, hairdryer, and clipper too close to the valve site.

As simple as it may be, complications and shunt infection rate are still high. Exercising caution and vigilant infection control aspects remain an integral part to the successful VP shunt surgery
