This gentleman has questionable cervical cord compression. He has hyperreflexia in the UE’s and LE’s and has a positive Hoffman’s sign, positive Babinski sign and lower extremity clonus which are signs of myelopathy and upper motor neuron lesion.

This is a 60y/o man with upper and lower extremity paresthesias. He is progressively losing balance and developing weakness in the left UE. He has a positive Hoffman’s sign, hyperreflexia, and clonus secondary to a severe central cervical spine stenosis.

If reflexes are hyperactive, test for ankle clonus.
ask the patient to relax.
support the knee in a partly flexed position.
quickly dorsiflex the foot and observe for rhythmic clonic movements.
Clonus is a sign of upper motor lesion and is usually associated with spasticity.
